| Adult Day Health Care/Adult Day Care | Adult Day Care
| The primary objectives Adult Day Health Care are to:
-restore or maintain optimal capacity for self-care to frail elderly persons and other physically or mentally impaired adults
-delay or prevent inappropriate or personally undesirable institutionalization

| Congregate Living Health Facility | Congregate Living Health Facility
| A Congregate Living Health Facility (CLHF) is a residential home with a capacity of no more than six beds, which provides 24-hour skilled nursing and supportive care. This care is generally less intense than that provided in general acute care hospitals but more intense than that provided in skilled nursing facilities. Congregate living health facilities provide at least one of the following services: Services for persons who are mentally alert, physically disabled persons, who may be ventilator dependent, persons who have a diagnosis of terminal illness and a life expectancy of six months or less, and persons who are catastrophically and severely disabled. |
| Home Health Agency | Home Care
| A Home Health Agency provides care to patients at their place of residence, at a level less intensive than health facility requirements. Services may include, but are not limited to, nursing care, intravenous therapy, respiratory/inhalation therapy, electrocardiogram, physical therapy, occupational and recreational therapy, and hospice services. |
| Hospice | Hospice
| A Hospice or Hospice Agency provides hospice services to terminally ill persons, is Medicare certified for hospice, and holds either a Hospice license or a Home Health Agency license from the California Department of Health Services. Any organizations, appropriately skilled professionals, or other professional persons or entities that are subcontracted by the hospice or hospice agency for the provision of specified hospice services to the resident are included within the definition. The hospice agency providing services in an RCFE shall not subcontracted with the licensee or any facility staff for the provision of services. |
| Nursing Homes/Facility | Nursing
| Nursing Homes provide care given to persons who are partially or totally unable to care for themselves, have health or mental problems and require convalescent and/or restorative services. Included are the following nursing services: skilled nursing care (SNF), intermediate care (ICF), mentally disordered care, developmentally disabled care, sub-acute care, hospice inpatient services, and other routine services. Also included are consultation and evaluation services. |
| Residential Care Facilities | Residential
| Residential Care Facilities for the elderly include housing arrangements chosen voluntarily by the resident, the resident's guardian, conservator or other responsible person. Seventy-five percent of the residents are 60 years of age or older and varying levels of care and supervision are provided as agreed to at time of admission or as determined necessary at subsequent times of reappraisal. Any younger residents in assisted living facilities must have needs compatible with other residents. |
