Types of Housing/Care Database Definitions
Adult Day Services Adult Day Care Adult Day Services provide programs during the daytime for adults with disabilities. Programs include activities, social interaction, nutritious meals, health screening and monitoring, personal care, and transportation. Respite for family caregivers is also available.
Assisted Living Residences Assisted Living Assisted living residence means a program or facility that combines housing, health and supportive services to support resident independence and aging in place. At a minimum, assisted living residences shall offer, within a homelike setting, a private bedroom, private bath, living space, kitchen capacity, and a lockable door. Assisted living shall promote resident self-direction and active participation in decision-making while emphasizing individuality, privacy and dignity.
Home Health Agencies Home Care Home Health Agencies provide health services in the home for elderly or disabled individuals. Services include: nursing, personal care, physical therapy, homemaker services, hospice care, and social work services. Thirteen non-profit Home Health Agencies are certified in Vermont. Medicaid Waiver funds may be used to pay for some Home Health Services. For services not covered by Medicare or Medicaid, cost is determined by a sliding fee scale.
Hospice Hospice Hospice provides physical, psychological, social and spiritual care for dying persons and their families. Services are provided by trained volunteers. Hospice also provides community education regarding loss & bereavement and direct services to bereaved persons.
Nursing Homes Nursing A Nursing Home means a place, other than a hospital as defined by statute, which maintains and operates facilities and provides nursing care for profit or otherwise, accommodating a person or persons, unrelated to the home operator, who are suffering from illness, disease, injury or deformity and require nursing care. Nursing Homes provide 24 hour nursing and personal care, supervision, hospice care, and rehabilitative care. Medicare pays for short term Nursing Home stays. Most Nursing Home care, however, is paid for by an individual's own funds or by Medicaid.
Residential Care Homes Residential A Residential Care Home is a place (excluding a licensed foster home) which provides, for profit or otherwise, room, board and personal care to three or more residents unrelated to the home operator. Residential Care Homes are divided into two groups, depending upon the level of care they provide, as follows: Level III which provides personal care, defined as assistance with meals, dressing, movement, bathing, grooming, medication, or other personal needs, or general supervision of physical or mental well-being. Level III includes nursing overview, but not full-time nursing care. Level IV which provides personal care, as described in a Level III facility, or general supervision of the physical or mental well-being of residents but does not include nursing care.
